A wide spectrum of social factors affect the health of individuals and families, and these factors are generally known as the social determinants of health. These factors include and are not limited to one’s level of education, their income, and the environment in which they live. When assessing the health needs of families, providers should consider these social elements, and their impact on the family.

The goal of such an assessment is to help providers to comprehensively understand the health needs of the family, and administer an intervention that meets the family’s health needs. It is imperative to assess how certain social factors affect the health of a selected family, propose age-appropriate screening, and develop the steps for health promotion.

The Social Determinants of Health

The family is doing well economically considering that the parents have well-paying jobs. However, this does not make the family immune to other social factors which can impact the health of families. Culture, and the beliefs which the family attaches to health, for example, seem to affect the family’s health status. The family holds values and beliefs which are rooted in the Christian doctrine of respect for life (Islam, 2019). Therefore, they maintain a hard stance against abortion. Another social issue is the access to the resources about health.

Effect of the Social Determinants of Health on the Family

The selected social factors seem to influence the family health in a variety of ways. The family beliefs, for example, can cause the family to refuse treatment that requires the termination of the pregnancy. These beliefs may cause problems if abortion is required to save the life of a member of the family such as the mother or the eldest daughter. The lack of access to the resources, still, can result to a situation in which the family does not get information on diseases such as diabetes and obesity.

Because of the lack of information on health management, the family may face the risk of obesity or other adverse health events (Khatiban, Tohidi, & Shahdoust, 2019). This social factor may have a serious impact on the health of the elderly in the family considering that she is vulnerable to health issues.

Why the Factors are Prevalent for the Family

There are key reasons why the social factors that affect health are prevalent in the family. First, each family has values and norms, and these are affected by their religious doctrines. Thus, the existence of these doctrines is what seems to cause the prevalence of the beliefs about health in the family. The issue of lack of resources is prevalent because of the lack of investment in such facilities in the community where the family lives (Richards et al., 2018). This can be attributed to the poor governance or the lack of the allocation of the adequate finances to fund the projects such as libraries.

Age-Appropriate Screening for Each Family Member

The needs of the family members may differ because of the age difference among members, and it is important to use the age-appropriate screening to evaluate the health needs of each member. The assessment for the 9-year old will entail examining the height, weight, and senses, and it is performed from the toe to the head. It will also involve examining whether the child can speak coherently.

The same assessment is used for the 12-year old. The assessment of the parents will involve gathering details about blood sugar, stress, and blood pressure. It will also involve prostrate screening and mammogram tests. The 66-year old will be screened cardiac issues and hypertension (Dorell et al., 2017).

Health Model for Creating the Plan

The action plan is based on the family health model (FHM). The goal of the action plan is to optimize the health of person in the family, and reduce the health risks for the family members. To achieve this goal, various measures will be undertaken. Such measures include educating the family about good health behaviors, and managing their health needs. The brochures and handheld devices will be used as the resources for educating the family. The reason for selecting the FHM is because the model provides a step-by-step process for addressing health concerns in the family.

Steps for the Family-Centered Health promotion and Communication Strategies

There are key steps which are involved in health promotion, and by following these steps, a provider can promote the health of the family. The first step entails letting the family to communicate their problems, and recognizing the role each person plays in terms of improving the wellbeing of the entire family unit. The second step entails involving the members in making decisions focused on improving the health of the family, before finalizing the process with the promotion of the cultural literacy (Zhang, 2018). When communicating with the family, strategies such as the use of brochures can help convey the message.

Conclusion

In summary, health assessment goes beyond looking for the physiological or psychological health needs. The assessment can help one gather the pertinent information about the family’s surroundings and other social factors which may affect the health of the family. After exploring the family health needs, providers may use the model such as the FHM to guide the intervention decisions.
